Nurse-led, not just coordinated

The healthcare system is fragmented, the appointments pile up, and the distance makes every silence feel ominous. You should not have to be the nurse, the scheduler, and the detective all at once. That is the job we take off your shoulders.
Subi is a registered nurse. She reads the labs, reconciles the medications, and reads the room with a clinician's eye, not a checklist.
We catch the small red flags before they become a 2am ambulance call. One avoided hospitalization more than offsets the cost of care.
You get one accountable RN care manager who knows your parent by name, not a rotating roster and a call center.
Deep, lived understanding of South Asian families and LGBTQ elders, where trust, dignity, and chosen family shape every care decision.
Who we walk beside

You live an hour or a flight away, you have your own kids and a job, and every call from your parent makes your stomach drop. We become your eyes, ears, and clinical judgment on the ground.

When the expectation is that you care for your parents yourself, bringing in help can feel like failing them. We hold that with you, with cultural understanding and zero judgment.

Chosen family, hard-won independence, and a healthcare system that has not always earned your trust. We advocate fiercely and meet you exactly as you are.
No long intake maze. We start with a conversation and move at the pace your family needs.
A care guidance call to hear what is going on and what is keeping you up at night. No script, no pressure.
A full clinical and home assessment of your parent: medications, conditions, safety, and the gaps no one has named yet.
Ongoing visits, medical coordination, and early-warning monitoring, with you kept in the loop every step.
As far as I'm concerned you're an angel that is heaven-sent, and I thank God for your healing spirit. You helped me understand how to manage my diabetes better and how to keep from having complications with my dialysis.
Subarna Mukherjee has been my mom's nurse for the last few years. She is a kind, dedicated, helpful person who has been willing to go that extra mile to help us meet the ever changing needs of our aging mother. I feel Subarna will be a great asset as you work to care for your aging family member too.
I am 91 years old, and Ms. Mukherjee has provided excellent services. Not only has she provided these services, but they have been administered with understanding and tact. This is very important to us older people.

A few sentences on Google helps the next family find a nurse they can trust.

Subarna Mukherjee is a master’s-prepared registered nurse (MPH, MSN) and Aging Life Care Professional who has championed community health from Honduras and India to the villages of Liberia. She built Shakti on a simple conviction: a nurse working alongside families in the home changes the whole trajectory of how an elder ages.
She is also a mother, a daughter, and a member of the sandwich generation. She knows the weight you are carrying, because she carries it too.
Common questions
A geriatric care manager (also called an aging life care professional) assesses an older adult's medical, safety, and daily-living needs, builds a care plan, coordinates doctors and services, and monitors for problems over time. At Shakti, that role is filled by a registered nurse, so the clinical judgment, medication management, and red-flag detection come built in.
For most families, yes. A single avoided emergency room visit or hospitalization typically costs far more than ongoing RN care management, and proactive oversight prevents many of those crises in the first place. The bigger return is often the family's time, sleep, and peace of mind.
Home care provides hands-on help with bathing, dressing, meals, and companionship. RN care management sits above that: a clinician assesses the whole situation, builds and adjusts the plan, coordinates every provider, and catches medical problems early. Many families use both, with the RN care manager directing the care.
We help families understand and manage a wide range of age-related and chronic conditions, including mild cognitive impairment and Alzheimer's, Parkinson's disease and other dementias, ALS (Lou Gehrig's disease), Huntington's disease, traumatic brain injury, stroke, cancer, congestive heart failure, chronic kidney and liver disease, COPD and emphysema, Type 1 and Type 2 diabetes, chronic pain, and severe anxiety or depression. As a registered nurse, Subi reads the labs, reconciles the medications, and coordinates the specialists so the whole picture is managed, not just one diagnosis.
Nurse-led care management is typically billed hourly, as prepaid hour packages, or through a monthly subscription, and is not usually covered by insurance or Medicare. Shakti offers tiered options, from prepaid hours and monthly monitoring to full concierge management, so families can match the level of support to their situation. We will walk you through exact pricing on your first call.
Generally no. Nurse-led care management and concierge nursing are private-pay services and are not typically covered by Medicare or health insurance. That independence is the point: we answer only to your family, not to a payer. Many families find the cost is more than offset by the hospital stays and complications it prevents.
Shakti serves Atlanta and the surrounding areas of Georgia with in-home nurse care management and concierge nursing. Remote care management is also available for special circumstances, so families outside the immediate area can still get clinical oversight.
