What we do

Proactive RN care management, built around one nurse who knows your loved one

Every family arrives somewhere different: planning ahead, managing a new diagnosis, or recovering from a hospital stay that went sideways. Choose the level of support that fits, and adjust as life changes.

What a nurse care manager actually does

  • In-home clinical and safety assessments
  • Medication reconciliation and management
  • Coordination across every doctor and specialist
  • Early detection of red flags before they escalate
  • Hospital discharge planning and transitions
  • Written care plans and regular family updates
  • Advocacy and shared decision making at the most critical junctures
  • Planning and connection to vetted service providers

One avoided hospital stay can cost more than a year of watchful care

Proactive RN care management is not another bill. It is the thing that prevents the bills, the falls, and the frantic phone calls. The return shows up as fewer emergencies and a family that finally sleeps.

Questions about cost and fit

Is a nurse care manager worth the cost?

For most families, yes. A single avoided emergency room visit or hospitalization typically costs far more than ongoing RN care management, and proactive oversight prevents many of those crises in the first place. The bigger return is often the family's time, sleep, and peace of mind.

What is the difference between home care and care management?

Home care provides hands-on help with bathing, dressing, meals, and companionship. RN care management sits above that: a clinician assesses the whole situation, builds and adjusts the plan, coordinates every provider, and catches medical problems early. Many families use both, with the RN care manager directing the care.

What health conditions do you commonly work with?

We help families understand and manage a wide range of age-related and chronic conditions, including mild cognitive impairment and Alzheimer's, Parkinson's disease and other dementias, ALS (Lou Gehrig's disease), Huntington's disease, traumatic brain injury, stroke, cancer, congestive heart failure, chronic kidney and liver disease, COPD and emphysema, Type 1 and Type 2 diabetes, chronic pain, and severe anxiety or depression. As a registered nurse, Subi reads the labs, reconciles the medications, and coordinates the specialists so the whole picture is managed, not just one diagnosis.

How much does a nurse care manager cost in Atlanta?

Nurse-led care management is typically billed hourly, as prepaid hour packages, or through a monthly subscription, and is not usually covered by insurance or Medicare. Shakti offers tiered options, from prepaid hours and monthly monitoring to full concierge management, so families can match the level of support to their situation. We will walk you through exact pricing on your first call.

Are your services covered by insurance?

Generally no. Nurse-led care management and concierge nursing are private-pay services and are not typically covered by Medicare or health insurance. That independence is the point: we answer only to your family, not to a payer. Many families find the cost is more than offset by the hospital stays and complications it prevents.
