 Each service visit includes:

•Head to toe assessment, VS monitoring, safety and risk assessment

•Medication management (pharmacy pickup, pillbox organization, refill management)

•Wellness and chronic disease management coaching

•Care coordination (appointment setup, review of notes and follow up on matters as needed.)

•Communication and feedback to family caregivers

 

Monthly, 6 month, or 12 month subscription options 

Care Management

A la carte services for care management can include:

​

  • Transition to Personal Care Home or Assisted Living

  • Hiring and Oversight of Private Caregivers

  • Accompaniment to Appointments

  • Post Hospital and Rehab Care Management

  • Medication Management

  • Caregiver and Dementia Coaching

  • Remote Monitoring Solutions

Concierge Nursing

​Older adults living with declining health may have many specialty appointments to navigate. With this service, an RN will:

​

  • ​Attend the appointment with your loved one

  • Serve as a liaison around any communication needs

  • Advocate for client preferences and goals of care

​​

Surgery is a risky time, especially for an elderly individual.  Even minor outpatient procedures can take a toll on one’s energy and result in complications.  Let us get everything set up for you and your loved one to have a smooth transition back home. We’ll make sure they:

  • have groceries

  • have all their durable medical equipment

  • get help with reconciling medications

  • get assistance obtaining transportation

  • get assistance with pre- and post- op procedures

  • get settled back at home 

  • get wellness checks to monitor for early signs of complications.  

If you or someone you love is having a procedure and you need help with any or all of the above, we got your back.
